Well, Alex, I'd basically consider unwanted and explicitly refused penetration to be sexual assault/rape, but perhaps I am naieve in my fairy-tale hopes and dreams that I will not be raped, although I know that too is not somehting that people usually plan to have happen to them.

Talking someone into it in the heat of the moment is one thing (because it involves consent), but I agree with the "accident" thing being sexual assault/rape, hence why the guy I know who tried that angered his GF immensely (though he didn't keep going with it).    I wouldn't say that you are naive, just there are some really fucked up people out there.   One would hope that women never get raped, but statistics clearly show us that is not the case.
