Serious Topic About Safety

We just had a mechanic crushed by an industrial truck he was working on when the driver didn't realize he was not clear.  One of the replies to the article in the newspaper was:

'Lockout-Tagout' safety ignored here, definitely.
 
The above policy is implemented by most all industrial / commercial concerns. 
 
The policy means that the truck's keys are in the hands of the mechanic and the steering wheel or gear shift is immobilized with a tag explaining who has them and why. The policy works.  
But it's too late for the mechanic here, RIP.


Those who have worked in industry may already know this policy.  But it is a good idea for everyone to at least leave a note tied to something you're working on to avoid even the most minor of accidents (turning on a faucet while the air's being bled from the pipes for example).

Couldn't help thinking about my friend who was crushed by a truck earlier this year. It wasn't a safety issue per se--he wasn't supposed to be there, he simply slipped--but close enough right now. :(

Where I used to work, the mushroom trayline used to have to be locked out if anyone wanted to work on it or clean inside it. One day I wanted to raise the stacker and destacker so I could clean better so I started their pumps - I didn't realise one of the guys I worked with was inside the compost hopper. He hadn't locked the machine out at all, it was still live. I could have started up the whole thing and he could have been badly hurt. It made me angry more than anything and I shouted at the guy that we have lock-out procedures (emergency stops and a padlock and key) for a reason.

My main occupational hazards are newt toxins.  Here's an example of how easily accidental poisoning can happen:
