I've got a psych appointment on Thursday with a new psychiatrist.  I wonder if this one will know his arse from his elbow; my experiences of doctors and psychiatrists so far is that they're often shockingly ignorant.  I found this exchange on Slashdot, and I'm wondering if I should print it out and take it along, since it would make a reasonable primer on 'ADHD: myths and misconceptions' if the psychiatrist turns out to be clueless
Sometimes psychiatrists don't like to read anything you bring in to them, they want to get your story first. I thought you had been diagnosed with ADHD?

I have known people in rl say that they don't believe in ADHD, my sister being one of them. But without the meds I wouldn't have a hope in going to uni.

My mom has adhd and she was able to raise two children. One of which was me, who had ODD and aspergers. Both constantly misdiagnosed my entire life.

I've been skeptical of ADHD since reading The Myth of the Hyperactive Child (Schrag/Divoky) and Talking Back to Ritalin (Breggin). Also like Thom Hartmann's POV: he sees ADHDers as being like hunters and innovators and NTs as farmers. He calls ADHD "the Edison gene."

BTW that's a very simplified xplanation of Hartmann's thesis. I'd read his books to learn more. Or listen to his radio show on Talk America.

Yes, I've been diagnosed with it, but the psychiatrists I've seen so far haven't been aware of how to treat it.  The first one I saw thought that since atomoxetine hadn't helped me, amphetamine or methlyphenidate wouldn't help me either, which was a load of bollocks.  None of them have had any experience of treating ADHD in adults, and little or no experience of it in children.  Their experience of stimulants has largely been limited to treating stimulant addicts, and when I see this new psychiatrist (new because my old one got ill and I've had a succession of locums since then), I don't want him to terminate my dexamphetamine prescription because he doesn't believe in ADHD and his only experience of amphetamine has been treating addicts with amphetamine-induced psychosis.  I've also been kept on a pediatric dosing schedule so far, since as far as I can tell, the NHS doesn't have a dexamphetamine dosing schedule for adults.

OMG that's backwards of the NHS. So no wonder that low dose doesn't help. Have you got a letter confirming your ADHD diagnosis from your old psychiatrist? Because that would have to be worth something.

You are right about some stimulants not working but others do. Ritalin does not work for me but dexamphetamine does. I have read and heard of this about other people as well.

I don't have a letter, but it should be in my medical notes, and the new psychiatrist will have those.  Atomoxetine was never likely to work for me in the first place, but it's the standard first-line drug that the NHS uses for ADHD these days, and from what I can tell, that decision was based entirely on it being a non-addictive non-stimulant, rather than it's medical efficacy, since it has half the rate of success of stimulants in clinical trials, a whole truck load of funky side-effects and costs far more than stimulants.  It was originally designed as an anti-depressant, so it's chemical similar to them, but was remarketed as a treatment for ADHD when it failed as an anti-depressant, and given that I've responded very badly to every AD I've tried so far, it didn't bode well for my response to atomoxetine, which turned out to be an extremely vile and horrible drug.

Right now I'm on 20mg a day of dexamphetamine, and the NHS guide says that care must be taken when going from 20mg to a maximum of 30mg, but in the adult clinical trials for dexamphetamine, the doses used were 40-60mg.  It doesn't help either that I only get to see a psychiatrist once every few months, and the dose has gone up in 5mg increments on those visits, when it's gone up at all, so it's taken a substantial length of time to get to even this low dose.

I have been prescribed o take up to 35mg a day. Since I haven't been working, I rarely take that much but I think it will go back up to that when I start uni full time. The amount was prescribed on how much I weigh. I am pretty small so I guess that is the reason for the 35mg limit which I am fine with.

I am wondering if you take that one amount all the time. Because you can take a day or two off them when you aren't doing much and this can help their effectiveness then. Otherwise you can get used to the dose and thus not feel as much benefit. I have days off from taking the meds and I just feel more tired and hungry. So a good sleep and eating a lot. :)

I always take the meds when I have to drive and/or go out and socialise. A good effect they have for me is they help make speaking easier.

I am also wondering if you can get a copy of the notes where it confirms you have ADHD. It would be good for you to have a copy of this in print for yourself.

We're not "handicapped" or "sick" (that goes for both AS and ADHD), just genetically different. But of course it's easier to stigmatize us than to say that we're as good or even better than the NTs and just genetically different. The Aspie "leave me alone" attitude isn't good for "society", because "society" demands that you'll be "social" in order to controll you.
