It means he is pretty significantly farsighted with mild astigmatism.

I had about that level of far-sightedness when I was three. And astigmatism, and a squint.

He sees far better than he sees close up, but not really well. At his age, the eyes are very adaptable but when he is older and if the far-sightedness remains, he will probably need reading glasses earlier than others.

Quick way to remember:

Far-sighted, you have your far vision (see things better that are far away)
Near-sighted, you have your near vision (see things better that are near by)

A good monarch is a treasure. A good politician is an oxymoron.

I think that preschoolers and younger children tend to be a bit far-sighted, though.

I know that when printing worksheets for younger children, you should use a bigger font than you would for older typical children.

True. And to not disturb the process of the eye developing towards less far-sighted, positive glasses prescribed for little children often are less positive than one would expect from the results. To keep the eye triggered to develop further. At least, that is what the ophthalmologist told me, couple of years ago. And a child with a far-sightedness fitting their age will not get glasses prescribed.
I don't know if they followed that rule for the urchin too. +4 is a lot.

He'll be fine. Kids have a wonderful ability to adapt. Not that he doesn't need glasses or that they won't help him, but right now he doesn't know any other way.
