I've had problems with a duodenal ulcer in the not-too-distant past, so I'd rather avoid ibuprofen.  The pain is caused by muscle spasms, which in turn is caused by something that's being investigated by a bunch of specialists at the hospital.  I'm waiting for the results of a few genetic tests at the moment.  The spasms last for 12 hours or more per muscle, and for the past month or two I've been in constant pain since at least a few muscles have been spasming at any given time.  One spasm caused an injury 21 months ago that still hasn't fully healed and causes quite severe pain at times.

How are your mineral levels? IIRC you do already take magnesium. What about the balance and amount of other minerals?

I don't know what my mineral levels are, but nothing like that has ever been flagged as a problem in the many blood tests I've had.  I sometimes take a combined calcium/magnesium supplement, but it can make me feel worse (or better, but more often worse) and generally makes my urine very cloudy, so I'm not routinely taking it at the moment.  I've got my hopes set on dopa responsive dystonia at the moment, since it's highly treatable and would explain both the physical issues and the depression, anxiety and ADHD.

I'm just about to.  I woke up again from the pain last night, but was too tired and in too much pain (it hurts more when I move) to get up and take paracetamol, so I just put up with it and tried to get a bit more sleep for an hour or two.

Any chance you store stress in your muscles?
I know I do that, sometimes enough to get incapacitated.
Physiotherapy works well for me. My therapist does now and then press me to take magnesium, because it makes it tangibly easier for her to get through the knots.
Have also had a splint for the worst arm muscle for a while, to force it to relax at night. Worked miracles.

If I am storing stress in my muscles, it's giving them a good workout; I saw the GP today and he was impressed with the muscle development in the regions where I get the most pain (I haven't been able to intentionally work out for the past couple of years for various reasons).  I told the GP that the co-codamol wasn't working for me, so he gave me ibuprofen and some proton pump blockers so that the ibuprofen wouldn't cause problems with what's left of the duodenal ulcer I had a year or two ago.  I asked him about muscle relaxants, but he didn't want to go down that route.

The combination of ibuprofen and paracetamol is working quite nicely so far.  I just hope my stomach and liver hold out.

I took my morning ones including the new one (fluoxetine). I have taken it in the past - it was the very first antidepressant I tried. It did nothing back then but then I was only on a low dose. I have to go back to the psych in a month to let him know how it is going.

You seem to have been stuck with a shitstorm of problems, I am sorry for that.
